At Phoenix Health, you as our member, are at the heart of everything we do; which is why we’ve partnered with Your Health Navigator to check in and ensure that you have access to the health support you need during these difficult times.
Your Health Navigator services
Your Health Navigator Registered Nurses help our members to not only navigate in‐hospital or in‐home treatment and provide complex health requirement care; their assistance can also extend to support services through these uncertain times, should you be awaiting elective surgery that has recently been postponed, or if you’re having difficulty accessing your usual health care services.
Here’s a few services provided by Your Health Navigator on behalf of Phoenix Health:

- Rehabilitation Programs
- Health Navigation Services
- Balance Program
- Social Support
Additional support for you when you need us
Together with Your Health Navigator, we have extended our existing services to our members directly impacted by COVID-19, because we want to support you as you continue to lockdown due to the pandemic. These services include, but are not limited to social support calls, and the opportunity to participate in our COVID‐19 online exercise program hosted by our registered Physiotherapists. This program can be provided to you one‐on‐one; or has also been designed for your whole family to participate.
