Getting to know you, and how you feel about us and the products and benefits we offer is important to helping us build a stronger health fund which can provide you with more value from your membership. At Phoenix Health, we believe health insurance should be easy to understand and use and feedback from our members is an important part to achieving this.
Our Research Partners
The Member Satisfaction Survey is hosted by Ipsos Pty Ltd; an independent research company who have been engaged to conduct this survey on behalf of the Members Health Fund Alliance, to which Phoenix Health is a member. The survey is conducted in line with the Phoenix Health Privacy Policy. Any information you share during the survey remains confidential and won’t be linked to your personal information.

Member Satisfaction Survey 2025

- These Terms and Conditions apply to Phoenix Health Fund’s ‘Member Satisfaction Survey October 2025’ competition. Instructions on how to enter and win prizes form part of these conditions of entry.
- The Promoter is Phoenix Health Fund ABN 93 000 124 863 of 1/ 4 Honeysuckle Drive, Newcastle NSW 2300. Phone 1800 028 817.
- Entry into this competition is deemed acceptance of these Terms and Conditions. To the extent of any inconsistency between these Terms and Conditions and any other reference to this competition, these Terms and Conditions will prevail.
- The competition is open from Tuesday 30 September 2025 and closes at 5.00pm (AEST) on Monday 20 October 2025. Any survey responses received after 5.00pm on 20 October 2025, or outside of this competition period will not be eligible for entrance into the draw to win.
- Prize includes one of five digital Your Choice Gift Card valued at $200 which can be redeemed online from any of the stores available at truerewards.com.au
- The competition is available to members who complete the Member Satisfaction Survey during the competition period and provide their member number on completion of the survey.
- The competition is not available to Phoenix Health Fund staff, immediate family, or Board Members.
- The ‘Member Satisfaction Survey October 2025’ competition is not valid in conjunction with any other offer or promotion.
- To be eligible to win, Members must be active and financial at the time of winning. Phoenix Health reserves the right to recover the value of the Gift Card should the Member or Membership be suspended or cancelled within three (3) months of the end of the competition.
- Winners will be drawn at random with an online generator in December 2025 at IPSOS pty ltd, 6/11 Wilson Street, South Yarra VC 3141. Winners will be notified by email following the draw.
- If any of the prizes are undeliverable or unclaimed 14 days following the draw date, as detailed above, no substitute prize or monetary recompense will be provided and a redraw will occur.
- Prizes will be sent by Phoenix Health to the Policy Holder on the Membership and delivered via email to the address listed on the Membership. Phoenix Health is not liable for lost, damaged or stolen Gift Cards and will not replace any Gift Cards in the event of loss, damage, theft or expiry.
- All prizes must be taken as offered and are not transferable, exchangeable or redeemable for physical Gift Cards or as cash.
- Phoenix Health have the right to amend these Terms and Conditions and may from time to time substitute or amend the reward for this promotion. Phoenix Health reserves the right to cancel this promotion at any time, without notice; or extend the promotion, at their discretion.
