Diabetes is a serious and chronic condition that affects over 1.5 million people in Australia – diabetes can seriously impact the quality of yours or your loved one’s life. To support our members health and wellbeing in managing this condition we have partnered with a couple of providers to deliver the Phoenix Health Diabetes Support Programs.

Diabetes Support Programs are for members who have been diagnosed with Type 2 Diabetes, or who have the risk factors of being pre-diabetic and a blood test shows that they’re borderline diabetic. They’re designed to help participants understand and manage life with Diabetes as well as the long-term risks associated with not managing their condition.

The program is available in face-to-face sessions, or remotely via tele health sessions and provides access to qualified Allied Health professionals who will provide support relevant to your situation and links you up with other services to ensure the care you receive is the best for your current situation as well as your future needs.

We know that no two people are the same, and what works for someone else may not necessarily be the best option for you. We’ve partnered up with the following providers, so that we can offer you a program that truly supports you and what you need right now…

Your Health Navigator (YHN)

Clinically led by YHN Diabetes educators, dieticians and registered nurses, the program offers five phone or telehealth sessions for members that are aimed at identifying their Diabetes goals and self-care plans and provide participants with a free blood glucose meter and the resources to give them the knowledge to understand their Diabetes to empower them to get the best possible results.

Doctors Kitchen

The Phoenix Health & Doctors Kitchen programs are an intensive 12-week plan, followed by 9 months of continued support and include:

  • A wellbeing assessment by a registered nurse
  • Free body composition scales and blood glucose monitor
  • Consults with a Doctor, Dietician and clinical team members
  • Meal plans, diabetes friendly recipes and shopping lists and resistance training plan with daily exercises

The program you are provided with will depend on your personal situation and are broken into the following levels:

  • Level One: Suitable for individuals who have been diagnosed as borderline or pre-diabetic or at high risk of developing Type 2 Diabetes
  • Level Two: Suitable for individuals who have been diagnosed with Type 2 Diabetes but who are not insulin dependant
  • Level Three: Suitable for individuals who have been diagnosed with Type 2 Diabetes and are insulin dependant

Designed to supplement any current care you’re receiving from your health professionals, the program will cover and include:

  • A Health assessment with a clinician who will tailor a program to your needs
  • Help understanding your Diabetes
  • The importance of nutrition and glucose levels
  • Keeping active and caring for your feet
  • Managing your medication
  • Caring for yourself and your Diabetes long-term.

Our Diabetes Support Programs are available at no extra cost to Phoenix Health members who have Hospital Cover, have served their 2-month waiting period and who have received a Type 2 Diabetes diagnosis and or meet the risk factors of being pre-diabetic (as diagnosed by a Doctor or by a blood test).
